+ CHRONIC WOUNDS

Wounds: An injury or break in the skin, generally a break in tissue or rupture of a membrane, due to some involuntary activity other than disease is called a Wound.

According to their general condition, location, the manner in which the skin or tissue is broken, duration and the agent that caused the wound, they are classified into 2 types:

  • Acute Wounds: Acute wounds normally proceed through an orderly and timely reparative process and heal within an expected time frame, which results in sustained restoration of anatomic and functional integrity.
    E.g. burns, cuts, surgical incisions, traumatic injuries, abrasions, minor skin irritants etc.
  • Chronic Wounds: Chronic wounds are wounds that does not heal within a standard time frame i.e. three weeks duration, these wounds may linger for weeks, months or even years.
    E.g. Diabetic Foot Ulcers, Venous Ulcers, Pressure Ulcers, Post-Surgical Wounds, Burns etc.
    • Diabetic Foot Ulcers: Diabetic Patients are particularly susceptible to developing ulcers due to two major reasons 1) Reduced sensation of the skin on the feet 2) Narrowing of the arteries going to the feet.
    • Venous Ulcer: Venous Ulcers usually occur in legs which are caused by improper functioning of tiny valves in the veins that normally prevent blood from flowing backward, which decreases normal blood circulation in the legs causing blood stasis, tissue damage & decreases wound healing.
    • Pressure Ulcers: Pressure Ulcers usually occurs in people who are bedridden or mobility is severely limited. It occurs when pressure on the tissue is greater than the pressure in capillaries, thereby cutting off circulation.
    • Post-Surgical Wounds: It is a physical injury during surgery to the body consisting of laceration or breaking of the skin/mucous membrane often with damage to underlying tissue.
    • Burns: It is a type of injury to flesh caused by heat, electricity, chemicals, light, radiation or friction. To distinguish a minor burn from a serious burn, the first step is to determine the extent of damage to body tissues.

Dosage:

  • Apply 1-2 times daily up to 50ml per treatment or as per the size of the wound bed.
  • Do not apply more than 120ml of solution within 24 hours.

Pressure Ulcer: 1-2 times a day, daily

Burns: Daily

I) For Doctors

  1. Is Ionic Silver Stable in the Solution?
    Yes
  2. Is there any burning sensation to the patients due to Silver/Menthol?
    No. In fact most of the patients found the effect of Qurion Soothing. A little cold sensation may be felt due to Menthol. In Qurion, Menthol not only serves as Penetration enhancer but also as Wound Odor Fighter
  3. Does Qurion have to be rubbed on the wound?
    Qurion is not to be rubbed on the wound. Qurion is to be sprayed on the wound through Syringe with 19-22G needle to create pressure of ~10 psi. Leave it on the wound for 10 minutes. After that soak the Gauze in Qurion Solution, apply it on the wound and complete the Dressing. This entire process holistically ensures disruption of Biofilm, killing of Bacteria and enhances healing.
  4. Does Qurion corrode or cause any damage to healthy, adjacent normal skin?
    No. Due to the lower concentration of active Silver Ions, no damage is done to the normal skin. Also, no pigmentation/darkening of the skin observed.
  5. Why 250 ml Bottle and not smaller pack?
    Each average sized wound treated with Qurion will require ~25 ml of Solution. 10 such treatments are minimally needed for elimination of colonization and wound healing. So, to increase compliance with visible success, Qurion is available in a minimal packing of 250 ml.
  6. What is the stability of Qurion Solution after opening bottle?
    28 days
  7. Does the syringe needle need to penetrate the wound?
    No. The Qurion solution is sprayed onto the wound keeping the syringe ~0.5-1.0 inch away.
  8. Can we first wash/irrigate the wound with Saline and then soak the wound with Qurion?
    As Saline does not aid much except mechanical debridement, it is not needed. In fact, use / presence of saline before / with / after Qurion may precipitate Silver ions in the solution and render it ineffective. This apart, irrigation with Qurion disrupts bacterial biofilm, which is not the case with saline.
  9. Does hypotonic solution help in removal of debris and necrotic tissue?
    Yes and that is why irrigation with Qurion is beneficial.
  10. Does surgical removal / sharp debridement of necrotic tissue needed along with Qurion therapy?
    Debridement in any form is symbiotic to use of Qurion and hence, surgical removal / Debridement of necrotic tissue is beneficial as cotherapy along with Qurion.
  11. Can Qurion be used with revision surgery?
    Qurion can be used in case of implants only while cleaning the opened area before inserting the implant in order to reduce the risk of colonization. In case of infected implant, before revision surgery, it is beneficial if the infected area is cleansed with the Qurion (spray and pour the Qurion Solution and keep it for 15 minutes) before implant is removed. Draining the solution and subsequent washing with saline or sterilized water is advised before proceeding with revision surgery.
  12. Can Qurion be used along with the implants?
    No. The reaction of Silver Ions with the implant material has not yet been tested and hence it is recommended not to use Qurion with the Implants.
  13. How is it proved that there is elimination of Infection?
    During the case studies conducted in the USA, the infection elimination and wound healing was judged on the color, odor, texture, tissue redness, formation of granulation tissues etc. No Biopsy/Lab Cultures were done, as these were only case studies and not clinical studies.
  14. What is the effectivitycriteria for treatment of Diabetic Foot Ulcers [DFUs]?
    51% wound size reduction in 5 weeks
  15. Can it be used for bladder irrigation in CAUTI cases [catheter associated urinary tract infections?
    Not yet tested, but theoretically must be possible to push in after verifying the results on optimization with amount, duration and frequency.
  16. How many times can the cap of Qurion bottle be punctured?
    As many as applicable during the course of treatment, within 28 days after the first puncture.
  17. Can it be used for Negative Pressure Dressings?
    Yes. Qurion can be used instead of saline for Negative Pressure Dressings.
  18. What is the reason for pain relief with QURION? Is it wound healing or presence of any analgesic in the solution?
    Apart from it penetration enhancing role, one of the main reasons for use of menthol is to bank upon its decades old pain relieving properties. Wound Healing and Menthol are 2 prime reasons for pain relief due to Qurion.

Result: It was seen that there is elimination of infection and 70% wound healing of the ulcer was observed within 40days of treatment
